Health Care Directive
Health Care Directive is a legal document that specifies what actions should be taken for your health if you are no longer able to make decisions for yourself because of illness or incapacity.

Durable Power of Attorney
In the event that you become mentally incapacitated, a durable power of attorney will allow you to choose someone to make decisions on your behalf. You will choose if they have the authority to make financial decisions, medical decisions, or both if there is an emergency that renders you unable to make them yourself.
